I, the undersigned parent or legal guardian, hereby acknowledge and agree that The Diva Den, its owners, employees and agents are not responsible for any injury, harm or accident that may occur to my child while at The Diva Den facility. I understand and accept that participation in activities at The Diva Den involves inherent risks, including the risk of physical injury. I voluntarily assume all such risks and waive any claims against The Diva Den for damages or injuries sustained during my child's participation. By signing this agreement, I agree to hold the company completely harmless, including financial responsibility for any injuries sustained, regardless of cause or circumstance. I will do all in my power to obey staff and all safety requirements, and will seek clarification if necessary.
